Compliance Worksheets

Parishes and schools in the Archdiocese of Milwaukee are required to complete annual documentation that outlines their compliance with our archdiocesan safe environment policies and procedures.  The Archdiocese of Milwaukee Safe Environment Office coordinates the collection of the annual documentation from each parish and/or school.  The information provided by parishes and schools is reviewed and verified with the pastor and on-site safe environment coordinator during any on-site Safe Environment Office review of the parish and/or school by the Safe Environment Office staff.  The Archdiocese of Milwaukee Safe Environment Office uses the information provided in the parish/school annual reports in their compilation of the annual USCCB Charter for the Protection of Children and Young People audit documentation. 

Parish Annual Safe Environment Compliance Report Form

Each year every parish and school in the Archdiocese of Milwaukee is required to submit an annual Compliance Documentation for Ensuring a Safe Environment  in the Parish and/or School.  This document, signed by the Pastor/Administrator/Parish Director ensures compliance with the United States Conference of Catholic Bishops’ Charter for the Protection of Children and Youth as applied in the Archdiocese of Milwaukee.  This form is due each year by July 1st.

Independent School Annual Safe Environment Compliance Report Form

Each year every school not associated with a parish in the Archdiocese of Milwaukee is required to submit an annual Compliance Documentation for Ensuring a Safe Environment  in the School.  This document, signed by the Principal, ensures compliance with the United States Conference of Catholic Bishops’ Charter for the Protection of Children and Youth as applied in the Archdiocese of Milwaukee. This form is due each year by July 1st.
Note: this document is only for schools that are not associated with a parish.  Schools affiliated with a parish will fill out the compliance documentation with their parish.
