Workers' Compensation Procedures

Following are the new worker injury reporting procedures with United Heartland Insurance Company.

Effective July 1, 2024, workers compensation claims are adjusted through United Heartland (AF Group).

When an employee experiences a work-related injury, the employee should immediately report it to the employer/supervisor.  Ideally the injured worker and their supervisor should make the reporting telephone call together right after the injury occurs and prior to seeking outside care.   If no supervisor is available, the injured worker should still place the call.

If the injury requires emergency treatment, call 911.

Injury reporting varies slightly based on whether treatment is required, or if you sought treatment prior to reporting the injury.

If no treatment is needed -OR- you have already sought treatment and just need to report the injury:

  • Call 1-866-323-4227 and press option 2
  • Provide Policy Number 1400023881
  • Make note of any claim or record number you may be given.

If you need to make a first report of injury and seek the assistance of a triage nurse:

  • Call 1-866-323-4227 and press option 1
  • Provide TCC Number: 12349
  • Make note of any claim or record number you may be given.

The triage nurse will report the injury to United Heartland.

The triage nurse will not advise you to go home or take the rest of the day off.

The triage nurse may recommend self-care and will offer to follow up with the injured worker in 24 hours. 

 -or-

The triage nurse may ask if you wish to have an in-person physician visit.  In this case, they will refer you to a preferred provider and will transfer your call to an authorized agent. This agent will ask you for personal information to set up the appointment and insurance billing.

 -or-

The triage nurse may ask if you wish to have a Telemedicine (virtual) visit.  You will be transferred to an authorized agent to set up the appointment.  Note: You must have a valid email address and device with a camera (such as a smartphone, tablet or laptop) to use Telemedicine.

The triage nurse will not ask you for personal information, such as your social security number, but the authorized agent will ask you for this information.

Claims incurred prior to July 1, 2024
