Instructions

  • Use the Form to the left  to apply for Religious Education Certification for teachers when they have completed the required sessions of Sustaining the Mission.
  • Only a qualified Educational Administrator may apply, that is, the school principal, parish religious ed director, or designated assistant.
  • You may submit names of up to 20 teachers who have earned the same level of certification.  When applying for more than 20 teachers, you must complete and submit the form more than once. 
  • When applying for teachers who have earned different levels of certification, submit a form for each level of certification.
  • To complete more than one form, after you receive the confirmation message "Thank you for submitting this application," you may use the Back button on browser to return to a blank form.
  • Records that verify completion of requirements must be maintained at the school, parish or institution.
