Transcripts or student records can only be issued to (a) the individual requesting his/her own record, (b) the legal guardian if under 18 years old, or (c) by a background check company with the individual’s signed release. Transcripts are not available to genealogists.
Requests made by companies or third-parties must be accompanied by an authorized release signed by the person (or legally qualified guardian) whose record is being requested.
How to Obtain Students Records
Active Schools: Please contact the school directly.
Closed/Merged Schools: In most cases if a school closed but the parish is still active, please contact the parish.
If a parish and school both closed/merged then record locations can be found below; please contact the appropriate school or entity. If records are held by the Archdiocesan Archives please complete the following:
- Submit a Student Records Release Request Form. Completed forms can be mailed, faxed or emailed to the Archives.
- Submit a copy of your government issued photo ID along with completed request form. We will not release records without proof of identification.
- Include a $10 processing fee. Payments can be made via cash or check made out to the Archdiocese of Milwaukee and must be mailed to the Archives.
